Members are required to carefully read the application form instructions and fill in
accurate information. Depending on the service item, please attach the application form,
affidavit, or consent form. All signature fields must be personally signed (any missing
signature will render the application invalid).
If any false information, impersonation, or illegal acts are found, the application will
be considered invalid and will not be processed. In cases of serious offenses such as
forged photos or signatures, IWPLAY World Interactive Entertainment Co., Ltd. reserves
the right to forward the applicant’s information to judicial authorities for legal
action.
If any corrections are made on the form, the account holder’s personal seal must be
affixed at the correction area.
Please attach a copy of valid identification for the account being applied for. For
detailed ID requirements, refer to the instructions on each form.
If the member is under 18 years old, a copy of the legal guardian’s ID must also be
attached, and the guardian must sign personally in the signature field.
If the information is inconsistent, documents are incomplete, or errors are found in the
form, the member will be notified to provide supplementary documents or resubmit. If
contact cannot be established within three days, the application will be considered
invalid.
Methods of submitting application documents: take a photo and upload it to the customer
service center (ensure that both the complete form and identification are visible), or
send by mail.
